Completing Assessments
Each assessment comprises three pages; Form 1, Form 2, and Summary.
Form 1
You may change the date of assessment here. Please read the guidance in the orange box.
For each item, rate the adolescent's strengths and vulnerabilities. You may click on the prompts to assist in making your assessment. Evidence supporting your assessment may be added to the notes section.
If the adolescent has strengths and/or vulnerabilities not covered in the provided items, you may add a Case Specific item at the bottom of the screen.
